Chronic Care Management

Chronic Care Management (CCM)

This programme is funded by Counties Manukau District Health Board and is available through all the MCHT general practices. People who are enrolled are entitled to a free G.P. and nurse visit every 3 months. Currently we provide this programme to those with diabetes but we are in the process of rolling out a new cardio vascular disease module.

Patients who are enrolled in the CCM programme receive a structure model of care and are very involved in their own care planning. Patients receive extensive education about their illness, how to take care of themselves, and how to access other health care providers who can assist them. Other ‘add-on’ services we can offer as part of CCM are: nutrition services, smoking cessation program, and an exercise programme.
